Membership Terms & Conditions
6-Month Commitment
All memberships require a minimum commitment of 6 months. Should you choose to cancel your membership before completing this period:
- Any accumulated banked funds will remain available for future use.
- Membership perks and discounts will immediately become void and ineligible for further use.
Freezing Memberships
Members may freeze their membership after fulfilling the initial 6-month commitment. To avoid re-enrollment fees, you must provide a future date to resume your membership when initiating a freeze.
Cancellation Exceptions
The following circumstances allow for cancellation without losing membership perks, pending appropriate documentation:
- Relocation due to military orders (military documentation required).
- Moving beyond a specified distance from the clinic (proof of new address required).
- Valid medical reasons preventing the use of services (physician’s note required).
Use of Banked Funds
- Banked funds may be used towards any available treatment or retail product.
- These funds are non-transferable but may be used to purchase gift cards; however, membership discounts do not transfer to gift card recipients.
- After cancellation, members have 90 days to utilize their banked funds at membership rates. Beyond this period, remaining funds will still be accessible but will be charged at non-member rates.
